Do I understand this right: Your healthcare insurance is linked to a bank account that you have to pay your health costs from and only when this exceeds a specified amount, the insurance will cover it?
As a comparison: Here in Germany the insurance costs 14.6% of your gross income. Your employer pays the half of it. Additionally the insurance companies charge an additional fee, see here for a list: krankenkassen.de/gesetzliche-k…
This covers all of your health expenses and you don't see any invoices or whatever. This is all done directly with the insurance company.
When you income is high enough, you can chose to be insured by a private company which mostly offers a better service.
