Sometimes the process of copying changes breaks, so a server that is queried by resolvers is out-of-date. This breakage is what these tools detect.
You cannot simply query all of the IP addresses from a single location to detect the breakage, since many DNS services use anycast, so the same IP address appears in many physical location.
