This was "solved" (or worked around) in b2g with signed apps.
Mozilla then just gave up, and now Google is pushing for "Isolated Web Apps" which are 99% similar to b2g security model.
I'm not sure if what Firefox does for WebMIDI is good enough - again it doesn't prevent service side compromise if I understand that solution correctly.
