My wife still forgets sometimes, but she's mostly learnt to push through the "he should just know" and tell me directly. Fortunately, it doesn't happen too often with her, but I've had it massively with previous relationships - partners, family and platonic. The only real solution, as far as I'm concerned is to talk about it. As I say, "it's never obvious to me", usually when I've missed a hint or misinterpreted something, and have upset someone.
