Vast swaths of what we have grown up thinking of as "churches" fall on a spectrum, having become partially or entirely apostate caricatures of the corporeal, living and breathing assemblies we are called to in the pages of scripture.
All the worldly notions of designing and implementing "authority structures," as you've so succinctly put it, have infiltrated and corrupted God's own body-design and turned that into a golem that would be totally unrecognizable to Peter, James, John, and Paul et. al.
A review of all the references you've cited reminds me that the very words of scriptur--as translated into English in most modern translations--have subtly and with malicious intent (that of our adversary, the devil) been collectively weighted towards obscuring the body-model of Jesus' congregation, replacing it with "structures" and "organizations" fundamentally no different from any garden-variety secular boardroom. "And there arose also a dispute among them as to which one of them was regarded to be greatest. And He said to them, “The kings of the Gentiles lord it over them, and those who have authority over them are called ‘Benefactors.’ But not so with you; rather the one who is the greatest among you must become like the youngest, and the leader like the servant. For who is greater, the one who reclines at the table or the one who serves? Is it not the one who reclines? But I am among you as the one who serves."
