Perhaps I've become a curmudgeon, but I no longer fill out a "tell us why you're unsubscribing from this email" boxes. I'm not doing their market research to cancel a service I no longer want (and sometimes never signed up for). If the systems requires you check something to unsubscribe, I check "other' and put NA.
