Well, I'm retired, and have not seen the patient, but I suspect that after examination and asking what that bit of body gets used for ("what do you do?") I'd tend toward a smear of Diclofenac or Ibuprofen gel over the sore bit and lump, a hot water bottle and some stretches for the muscle, and some thoughts about webbing or harnesses for carrying...
And more often than many colleagues, an injection of a drop of local anaesthetic and some weak steroid into the pain/lump.
