I'm very bad at heel+toe, so it's easy to just whack the throttle so it smacks the limiter, then as it drops you time the clutch engagement so it meshes when the revs are matched.

In a perfect world you'd give it just enough throttle to match revs - but that is extremely difficult when there's no load on the engine. So I do it the simple way.
