Probably having the same problem I have off and on over the years - getting booted off by really conservative instance admins, or servers going down without warning. 😐
I at least have learned to back up my follow list weekly so I don't lose track of who I followed if a server dies. You can upload the list to your new server in seconds and immediately be following almost everyone you were before (minus the massive inconvenience of servers that block each other).
