you are missing something, just because you as a user can't figure out where the version of tar on your rootfs came from without going back to where you got the OS image from doesn't mean there's no package manager. It just means the package manager is external to the system. Somewhere, someone needed to fetch the tar sources, build them, and add them to the image. GNOME OS just takes away the ability to perform introspection after the fact.
The actual difference between GNOME OS and a more traditional distro like Alpine is that you build the thing entirely from source, which is great if you're Google making Android, but not so much for small communities to maintain.
This is what I meant by "magical alternative way to build a system", there isn't one. You see GNOME OS and assume that they have one, they don't, they're just taking away user control under the guise of immutable simplicity / stability.
With postmarketOS, I plan to provide exactly the same level of immutable simplicity / stability, but with APK's local package database left intact, so you can mount a mutable sysext and then go tinker and do whatever you want on your system. On the other hand, GNOME OS plans to solve this by [shipping sysext images for developers](https://discourse.gnome.org/t/towards-a-better-way-to-hack-and-test-your-system-components/21075 ).
