I was wondering the same thing. It seems that an uncensorable decentralized chat protocol will certainly have spam. I assume there will be the ability to block at some point.
But when you look at the private/public keys as ID + relays with no single point of failure you have a social media without any rulers or anyone able to shut anyone down.
So whatever spam arises seems like a reasonable trade off.
