I just lost 94 days of progress on a Stack Overflow achievement because it didn't count me visiting the site yesterday, even though it's clearly visible in my Firefox history at 5:31 PM on 6/28. (Days are counted in UTC, so 5PM for me is midnight UTC.) It did count correctly on two other sites, Super User and Server Fault, just not SO. There's no other explanation than a defect in Stack Overflow. Thanks for the bug you moronic amateurs!
